About Zona Physical Therapy & Wellness
Zona Physical Therapy & Wellness is a locally owned outpatient healthcare practice serving Buford, Gwinnett County, and surrounding communities.
We provide individualized physical therapy services for patients recovering from pain, injury, surgery, neurological conditions, balance problems, pelvic floor dysfunction, workplace injuries, and motor vehicle accidents.
Our physical therapy services include:
- Orthopedic and musculoskeletal rehabilitation
- Back, neck, shoulder, hip, knee, foot, and ankle rehabilitation
- Post-surgical physical therapy
- Neurological rehabilitation
- Balance and fall-prevention therapy
- Women's health and pelvic floor physical therapy
- Motor vehicle accident rehabilitation
- Workplace injury rehabilitation
Our ownership team is also exploring expansion into home health care and other healthcare services. The future home health operation may be developed as an independent brand, an affiliated company, or through a franchise relationship. The final name, structure, and launch timeline have not yet been determined.
We are seeking a Healthcare Outreach Liaison who can initially support the growth of Zona Physical Therapy and potentially expand into a broader liaison role as additional healthcare services are introduced.
Position Overview
Zona Physical Therapy & Wellness is seeking an outgoing, dependable, and relationship-focused Healthcare Outreach Liaison to represent our practice throughout the local healthcare community.
The Healthcare Outreach Liaison will develop and maintain relationships with physician offices, hospitals, medical practices, discharge planners, case managers, attorneys, employers, senior communities, and other potential referral partners.
The initial focus of this position will be increasing awareness of Zona Physical Therapy's services and strengthening our patient referral network.
As our ownership group expands into home health care or other healthcare services, the successful candidate may be offered additional hours and responsibilities supporting those new service lines. This could include representing a separately branded, affiliated, or franchised home health organization.

Key Responsibilities
- Conduct in-person visits to physician offices, specialty practices, hospitals, urgent care centers, and other healthcare facilities.
- Introduce Zona Physical Therapy and explain our clinical services, specialty programs, and referral process.
- Build professional relationships with physicians, referral coordinators, office managers, nurses, case managers, discharge planners, and other healthcare professionals.
- Educate referral partners about the patient populations and conditions treated at Zona Physical Therapy.
- Deliver brochures, referral forms, service information, and other approved outreach materials.
- Follow up consistently with existing and prospective referral partners.
- Identify new referral and partnership opportunities throughout Gwinnett County and nearby communities.
- Schedule and coordinate office presentations, provider meetings, lunch-and-learns, and community outreach activities.
- Develop relationships with hospitals, senior living communities, employers, attorneys, workers' compensation professionals, and community organizations.
- Represent Zona Physical Therapy professionally at healthcare networking events, health fairs, community programs, and local business events.
- Track office visits, contacts, conversations, follow-up activities, and referral opportunities using the company's designated tracking system.
- Provide regular updates to leadership regarding outreach activity, referral trends, market feedback, and partnership opportunities.
- Assist with the development and improvement of outreach materials, referral resources, and territory plans.
- Maintain a professional understanding of Zona's services, scheduling process, referral procedures, accepted insurance plans, and target patient populations.
- Protect confidential business and patient information and follow all applicable privacy and professional standards.
Potential Future Healthcare Outreach Responsibilities
As the ownership group develops or acquires additional healthcare services, this position may expand to include outreach on behalf of a future home health organization or other affiliated healthcare operation.
Potential future responsibilities may include:
- Introducing home health services to physicians, hospitals, rehabilitation facilities, senior living communities, and community partners.
- Building relationships with hospital discharge planners, case managers, and care-transition teams.
- Developing relationships with skilled nursing facilities, assisted living communities, primary care providers, and specialists.
- Educating referral sources about home health eligibility, available services, coverage areas, and referral procedures.
- Supporting referral development for skilled nursing, physical therapy, occupational therapy, speech therapy, and home health aide services.
- Following up with referral partners regarding service availability and patient access.
- Helping establish a new home health organization's presence within the local healthcare community.
- Supporting outreach across multiple healthcare brands or service lines when appropriate.
Any future duties, hours, compensation changes, or reporting relationships will depend on the structure and needs of the new healthcare operation.

- Availability during weekday business hours when physician offices and healthcare facilities are open.
Preferred Qualifications
- Previous experience in healthcare outreach, physician relations, medical marketing, provider relations, business development, community outreach, or medical sales.
- Experience working with physical therapy, rehabilitation, home health, hospice, senior care, medical practices, hospitals, pharmaceutical companies, or medical device organizations.
- Existing professional relationships within Gwinnett County or the surrounding healthcare community.
- Familiarity with healthcare referrals and physician office operations.
- Experience organizing lunch-and-learns, provider presentations, networking events, or community programs.
- Knowledge of physical therapy or home health services is helpful but not required.
- Experience developing a new healthcare territory or referral network is a plus.

This position is intended to begin as a limited part-time role. As referral activity grows and additional healthcare services are launched, acquired, or added, the position may increase to a larger part-time schedule or develop into a full-time healthcare outreach, referral development, or business development role.
An increase in hours or transition to full-time employment is a potential growth opportunity and is not guaranteed by a specific date.
Compensation
- $25.00–$35.00 per hour, depending on relevant experience and established healthcare relationships

Outreach performance will not be measured solely by the number of brochures distributed or offices visited. The quality of relationships, consistency of follow-up, referral growth, and long-term partnership development will also be considered.
Initial 90-Day Priorities
During the first 90 days, the Healthcare Outreach Liaison will be expected to:
- Learn Zona Physical Therapy's services, referral process, target patient populations, and service area.
- Become familiar with approved outreach materials and talking points.
- Create or refine a targeted list of local referral sources.
- Establish an organized weekly outreach schedule and territory route.
- Begin making consistent visits to physician offices and healthcare organizations.
- Document contacts, conversations, opportunities, and required follow-up.
- Reconnect with inactive or underutilized referral sources.
- Identify lunch-and-learn, presentation, and partnership opportunities.
- Provide leadership with regular feedback regarding referral-source needs and market opportunities.
- Demonstrate professional, consistent, and reliable follow-through.

Work Setting and Travel Requirements
This is a local, in-person, field-based position. It is not a remote-only marketing position.
Candidates must live within reasonable driving distance of Buford, Georgia, and must be comfortable traveling throughout Gwinnett County and surrounding communities.
Some administrative work, reporting, scheduling, and follow-up may be completed remotely or from the clinic. However, the primary responsibility of the position is conducting in-person healthcare and community outreach.
