Occupational Therapist – Skilled Nursing Facility | Menlo Park, CA

We are partnering with a well-established 160-bed post-acute and skilled nursing facility in Menlo Park, CA to find an experienced, compassionate, and professional Occupational Therapist (OT) to join their growing rehabilitation team. This is a permanent, full-time opportunity with an ASAP start — ideal for a clinician who thrives in a collaborative SNF environment.

About the Role

As an Occupational Therapist, you will evaluate and treat a geriatric patient population recovering from surgery, illness, or injury in a post-acute setting. You will manage an average caseload of 10 patients per day, with a productivity expectation of 87–90%. The facility uses PCC and Net Health as their EMR systems.

You will be part of a team of 5 therapists in a flexible, cooperative environment where collaboration and patient-centered care are at the heart of everything they do.

Key Responsibilities

• Evaluate and develop individualized occupational therapy treatment plans for geriatric patients
• Deliver skilled therapeutic interventions targeting ADLs, IADLs, fine motor coordination, cognitive function, and adaptive equipment needs
• Conduct home safety evaluations and recommend appropriate adaptive strategies and equipment
• Maintain 87–90% productivity while delivering high-quality, patient-centered care
• Accurately document patient progress and care plans in PCC and Net Health
• Collaborate with an interdisciplinary team including PTs, SLPs, nursing, and physicians
• Supervise Certified Occupational Therapy Assistants (COTAs) as applicable
• Participate in discharge planning and care coordination

Requirements

• Current California OT license (or eligibility to obtain)
• Graduate of an accredited Occupational Therapy program (MOT or OTD preferred)
• Minimum 1–2 years of SNF or post-acute experience required
• Proficiency or willingness to learn PCC and Net Health EMR
• Strong compliance, professionalism, and communication skills
• Team-oriented, flexible, and patient-focused work style
