We are partnering with a well-established, 18-year-old integrated wellness center in the Rockford/Loves Park, IL area to find a skilled and passionate Physical Therapist (PT) to join their multidisciplinary team. Position Details

• Setting: Integrated Wellness Center — Pain & Wellness Clinic (Outpatient)
• Patient Population: Adults — musculoskeletal, pain management, rehabilitation
• Schedule: Full-Time or Part-Time | Flexible | Hourly or Salary
• Location: Loves Park, IL (Rockford area)
• Compensation: $45-50/hr, DOE | Hourly or salary structure | Contract required
• Start Date: ASAP

About the Role

The Physical Therapist assesses the need for, develops, and delivers individualized physical therapy programs to facilitate patient rehabilitation. You will identify the need for referrals to other disciplines and implement therapy programs designed to restore, reinforce, and enhance patient performance. Working within a supportive, multidisciplinary team environment, you'll collaborate with chiropractors, massage therapists, and a Nurse Practitioner to deliver truly integrated, patient-centered care.

Key Responsibilities

• Deliver effective and efficient rehabilitation services in adherence to established plans of care, schedules, and documentation requirements
• Screen, examine, and evaluate patients including history, systems review, and appropriate tests and measures
• Formulate diagnoses, identify impairments, predict level of improvement, and design individualized plans of care
• Develop appropriate treatment goals in collaboration with patients and caregivers and implement physical therapy treatment plans
• Provide skilled therapeutic interventions including:
  • Therapeutic exercise (aerobic conditioning, balance/agility training, body mechanics, flexibility, gait/locomotion, neuromotor development, strength/endurance)
  • Functional training
  • Manual therapy
  • Airway clearance techniques
  • Use of electrotherapeutic, physical agent, and mechanical modalities
• Adapt programs and plans of care to meet individual patient needs
• Make referrals to other disciplines and agencies to address comprehensive patient needs
• Consult with other rehabilitation team members to select the most appropriate therapeutic regimen
• Attend and contribute to patient care and staffing conferences
• Provide patient and caregiver education on skills, home programs, and independence-building activities
• Promote a safe environment and uphold patient rights and dignity
• Complete accurate and timely documentation of all services provided

Requirements

• Physical Therapy License — Illinois license preferred
• CPR Certification — preferred
• Strong interpersonal and communication skills
• Collaborative, team-oriented mindset
• Passion for holistic, patient-centered wellness care
