RN Case Manager – Home Health | Full Time
Pay Range: $140,000 – $160,000 per year (DOE)
Alternative Pay Option: $130,000 – $150,000 per year + Company Vehicle
Schedule: Monday–Friday, Full-Time
Coverage Area: Santa Clara County with occasional coverage in Alameda County
Position Summary
We are seeking an experienced and patient-focused RN Case Manager to join a 5-star, clinician-led home health organization serving Santa Clara and Alameda Counties. In this role, you will partner closely with leadership and interdisciplinary teams to manage patient care, ensure clinical excellence, and support high-quality outcomes across each episode of care.

Patient Population
- Primarily geriatric patients in a home health setting
- Individuals requiring skilled nursing, chronic disease management, and care coordination
- Collaboration with caregivers, families, and interdisciplinary care teams
Key Responsibilities & Essential Functions
Patient Care & Case Management
- Manage a caseload of home health patients across Santa Clara and Alameda Counties
- Conduct comprehensive Start of Care (SOC) and ongoing nursing assessments
- Develop, implement, and oversee individualized plans of care
- Identify changes in condition early and adjust care plans to prevent hospitalizations
Clinical Care & Skilled Nursing
- Perform in-home visits providing skilled nursing care and assessments
- Support patients with complex medical conditions and post-acute needs
- Deliver high-quality, patient-centered care with a focus on outcomes
- Ensure continuity and quality across the full episode of care
Care Coordination & Interdisciplinary Collaboration
- Coordinate care across PT, OT, ST, MSW, HHA, and other team members
- Serve as the primary clinical liaison between field staff and leadership
- Communicate effectively with physicians, caregivers, and internal teams
- Ensure seamless coordination and continuity of care
Clinical Leadership & Team Support
- Mentor and support field clinicians to promote best practices
- Provide guidance on clinical decision-making and patient management
- Partner with leadership to maintain high standards of care delivery
- Contribute to the development of clinical processes and team performance
Documentation & Compliance
- Complete accurate, timely, and compliant documentation
- Review and ensure accuracy of OASIS assessments and submissions
- Maintain compliance with Medicare home health regulations and agency standards
- Utilize WellSky (Kinnser) EHR effectively for documentation and care tracking
Patient & Caregiver Education
- Educate patients and families on disease management, medications, and safety
- Reinforce care plans and promote adherence to treatment goals
- Support long-term wellness and independence in the home setting
Qualifications
Education & Experience
- Minimum 3 years of professional nursing experience (Required)
- Experience in home health or community-based care (Preferred)
- Case management experience (Preferred)
Licensure & Certifications
- Active California RN license in good standing (Required)
- Current CPR certification (Required)
