  1. B.S. in Pharmacy or Pharm.D. (required).
  2. Minimum of 1 year as a Pharmacy Clinical/Operations Specialist.
  3. Active Pharmacist license in good standing with the State Board of Pharmacy.
  4. Active Pharmacy Consultant license (or state equivalent).
  5. 5+ years of hospital pharmacy experience, with demonstrated expertise in the area of focus.
