The Role
We are seeking a detail-oriented and proactive Intake/Referral Coordinator to join our team. In this vital role, you will serve as the initial point of contact for new referrals, ensuring seamless transitions into our care. Your work will help us connect patients to the services they need, contributing to the quality care and support that We Care Home Health is known for.
What You'll Be Doing
- Receive, evaluate, and process all incoming referrals with accuracy and efficiency.
- Complete referral documentation, including medical and demographic information.
- Verify insurance coverage and obtain prior authorizations from third-party payers.
- Coordinate with clinical and scheduling teams to ensure timely initiation of care.
- Act as a resource for families, clients, and referral sources, providing information on available services and coverage options.
- Maintain accurate logs, files, and records to ensure compliance and follow-up on unresolved coverage issues.
- Collaborate with internal teams, including marketing, clinical, and accounting staff, to educate on insurance benefits, resources, and community support.
- Participate in quality improvement activities to enhance the intake/referral process.
What Makes You a Great Fit
- Minimum of one year of experience in intake, referrals, insurance authorizations, or a related role preferred.
- Familiarity with community resources, Medicare/state regulations, and third-party/private payer reimbursement criteria.
- Excellent communication skills (verbal and written), with a focus on professionalism and teamwork.
- Strong attention to detail, critical thinking, and time management skills.
- Ability to work independently while managing multiple tasks efficiently.
- Proficiency in computer data entry and documentation.
