The Role
Were looking for an energetic and compassionate Registered Nurse (RN) to join our growing team! You will play a critical role in welcoming new patients into our care by completing OASIS Start of Care assessments and developing personalized care plans. This role offers the flexibility of working on intermittent visits, giving you the chance to make a meaningful impact on each patients life.
What You'll Be Doing
- Conduct comprehensive Start of Care (SOC) OASIS assessments to create individualized care plans and complete Recertifications or Discharges as needed
- Provide hands-on nursing care, including administering medications, wound care, catheterization, tube feedings, trach/vent care, and various additional nursing tasks
- Build meaningful relationships with patients and their families, guiding them through their care journey
- Collaborate with a supportive and experienced team of healthcare professionals to ensure coordinated care
- Document patient care and progress using the KanTime EHR system
- Contribute to our mission of providing superior quality care that exceeds expectations
What Makes You a Great Fit
- Current Colorado RN license
- Graduate of an accredited nursing program
- OASIS experience
- Current BLS/CPR certification
- Strong communication and clinical assessment skills
- Ability to work independently and as part of a collaborative team
- Previous home care experience is a plus
