Responsibilities:
As our Primary Care Physician, you will be the heart of a powerful, interdisciplinary care model. You'll perform in-person comprehensive assessments for our participants, partnering closely with the Interdisciplinary Team (IDT) to craft individualized, life-changing care plans.
Working alongside and under the leadership of our inspiring Medical Director, you oversee participant care across nursing homes and hospitals, handling admissions, discharges, and coordinating short-term, long-term, and acute placements. Your strategic use of medical specialists ensures each participant receives seamless, effective care every step of the way.
This is more than just a clinical role it's leadership in action. You'll also mentor and guide Nurse Practitioners and support IDT members, helping elevate standards of care across the board.
- Conduct thorough in-person assessments and develop customized care plans.
- Coordinate care across multiple settings including nursing homes and hospitals.
- Manage admissions and discharges for all placement types: acute, short-term, and long-term.
- Leverage a network of medical specialists to ensure the highest quality outcomes.
- Lead, support, and mentor Nurse Practitioners and other clinical team members.
- Ensure care strategies align with the Medical Directors vision and organizational standards.
Qualifications:
- Board Certified in Family Medicine, Geriatrics, or Internal Medicine
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Masters Degree/ Doctorate Degree
- Active California Physician License without Restrictions
- State CDS Certification and DEA Registration
- Valid State Drivers License with a clean DMV record
- Copy of recent Vehicle Insurance required
Essential Duties:
- Provide assessment, planning, primary care, and continuous medical coverage to participants
- Collaborate with community-based physicians and IDT in discharge planning from acute or long-term settings
- Actively participate in IDT, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery
- Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
- Take part in rotating night and weekend call
Job Requirements:
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Masters Degree/ Doctorate Degree
- Board Certification Internal Medicine or Family Practice
- Board Certification Geriatrics preferred
- Active NY Physician License without Restrictions
- State CDS Certification and DEA Registration
- Valid State Drivers License with a clean DMV record
- Copy of recent Vehicle Insurance required
- less than 1 year experience in specialty, new graduates considered
