Declaration and Consent for Use of Personal Data

I hereby declare that the information provided in this application and any supporting documents is true, accurate and complete to the best of my knowledge. I understand that any false statement, misrepresentation or omission may result in the rejection of my application or, if employed, termination of my employment.

In accordance with the Personal Data Protection Act 2012 (PDPA) of Singapore, I consent to Vertis Digital Pte. Ltd. collecting, using, disclosing and processing my personal data for purposes relating to recruitment, administration and evaluation of my suitability for employment. This includes, but is not limited to:

• Contacting me regarding job opportunities;
• Conducting reference, qualification and background checks;
• Processing and evaluating my application;
• Internal HR and operational purposes.

If I am offered and accept employment, I further consent to the disclosure of my personal data to:

• The Company’s clients for onboarding, background screening or project deployment purposes;
• Third-party service providers (including payroll, insurers, IT systems and background screening vendors);
• Regulatory or government authorities, whether in or outside Singapore, where required by law.

Where personal data is transferred outside Singapore, Vertis Digital will take reasonable steps to ensure that the recipient provides a standard of protection comparable to that under the PDPA.

I understand that my personal data may be retained for future job opportunities unless I withdraw my consent.

I may request access to or correction of my personal data, or withdraw my consent, by contacting agalya@vertis.digital. Withdrawal of consent may affect the Company’s ability to process my application.
