I hereby affirm that the information provided on this application (and accompanying resume, if any) is true and complete to the best of my knowledge. I also agree that falsified information or significant omissions may disqualify me from further consideration for employment and may be considered justification for dismissal if discovered at a later date.

I authorize all persons listed above (and on the accompanying resume, if any) to give USALCO any and all information concerning my previous employment and education and any pertinent information they may have, personal or otherwise, and release all parties, such persons and USALCO from liability for any damage that may result from furnishing same to USALCO.

I understand that, subject to applicable state laws, USALCO may require as a condition of employment that a background check be conducted at the time a conditional offer of employment is made. If a conditional offer of employment is made, I will be provided with a Release Authorization, and will be required to return the signed form to USALCO. I further understand that I may be required to take, at company expense, a drug test and/or a physical exam to determine whether I a am physically able to perform the functions of the job.

I understand that the position for which I am applying is at-will in nature, meaning that if I am hired, either I or USALCO may end the employment relationship at any time, for any reason.
