Are you a seasoned professional in healthcare management, passionate about leading impactful case management programs? We are seeking a dynamic and experienced Director of Case Management to oversee and implement our robust case management program at the local level.

Responsibilities:

  • Drive the implementation and execution of the inpatient case management program, overseeing Care Facilitation, Utilization Management, Case Management, and Discharge Planning.
  • Ensure compliance with industry standards, regulations, and best practices, optimizing patient care quality and cost-effectiveness.
  • Collaborate closely with interdisciplinary teams to streamline processes, enhance patient outcomes, and facilitate seamless transitions of care.

Qualifications:

  • Current and valid license: Registered Nurse in the state of Texas (or compact state) or Masters Social Worker (LMSW) in Texas. LCSW preferred.
  • Certification: Certified Case Manager (CCM), Accredited Case Manager (ACM), or Fellowship of the American Academy of Case Management (FAACM) required.
  • Experience: Minimum five (5) years in utilization management, case management, discharge planning, or other cost/quality management programs. Three (3) years of hospital-based nursing or social work preferred.
  • Supervisory experience: Demonstrated leadership with at least 5 years of supervisory experience.
