Key Responsibilities:

As the Director of Case Management, you will play a pivotal role in our hospital's strategic plan by:

• Conducting comprehensive assessments of individual patient needs.
• Developing and implementing patient care plans.
• Overseeing utilization review and management of discharge planning.
• Ensuring the highest level of patient satisfaction and continuity of care.
• Coordinating and managing the care and discharge planning for all patients.

Qualifications:

Education:

• Graduate of a Program in Nursing at the Bachelor of Science level or higher (Master's degree preferred).
• CCM or ACM Certification is preferred.

Skills:

• Critical thinking and problem-solving abilities.
• Strong interpersonal and communication skills.
• Proficiency in data analysis, report generation, and budget management.
• Organizational leadership and management skills.

Work Experience:

• A minimum of five (5) years of management experience.
• Five (5) years of utilization review/case management experience in an acute short-term hospital.
• Five (5) years of experience in discharge planning in an acute short-term hospital.
• Experience with Interqual or MCG criteria software, Microsoft Word, Excel, and PowerPoint.
• Preferred experience with MedHost EMR and MorCare.

Licenses:

• Registered Nurse with current RN Licensure from the State of Arizona or compact license accepted by the State of Arizona.
• Basic Life Support (BLS) certification.

If you don't already have Case Management certification, don't worry; we support your professional growth. You'll have the opportunity to sit for the appropriate certification within 30 months of hire, provided all other criteria are met.
