Job Summary:

We are seeking a detail-oriented Remote Home Care Intake Specialist to manage the full intake process for Home Care and MLTC/MAP patients, ensuring accuracy, compliance, and timely enrollment. The role includes explaining MLTC/MAP enrollment to patients, scheduling NYIA assessments, referring patients to appropriate plans, following up on UAS and enrollment statuses, and assisting with plan-to-plan transfers. The Intake Specialist will verify Medicaid eligibility, maintain case records in Boelt/BOLT CRM, track and report data, and coordinate daily communications, all while operating in a fast-paced environment and proactively resolving obstacles. This position requires knowledge of Medicaid workflows, NYIA assessments, MLTC/MAP plans, and the ability to navigate complex Home Care intake beyond simple data entry, maintaining confidentiality and high attention to detail. In this role the candidate will have a strong English communication and interpersonal skills. Prior experience in intake, eligibility, or Medicaid-focused case management is required, along with familiarity with ePACES or similar platforms and Boelt/BOLT CRM. Knowledge of MLTC/MAP plans, NYIA assessments, Medicaid codes, and Home Care intake processes is essential. Strong problem-solving, multitasking, organizational skills, and proficiency in Microsoft Office and Google Workspace are preferred. Candidates must demonstrate sound judgment, attention to detail, and the ability to follow processes and learn new systems with minimal supervision.

Key Responsibilities:

  • Respond to customer inquiries via phone, email, or chat.
  • Explain the MLTC/MAP enrollment process to patients.
  • Schedule NYIA assessments with patients.
  • Refer patients to appropriate MLTC Plans.
  • Follow up with MLTC Plans for updates on UAS (Uniform Assessment System) and enrollment statuses.
  • Assist patients with plan-to-plan transfers.
  • Conduct detailed intake for Home Care, which may vary from quick 48-hour enrollments to more complex, lengthy processes.
  • Operate effectively in a fast-paced environment and proactively resolve obstacles during the enrollment process.
  • Manage the full intake process for incoming cases, ensuring accurate tracking, reporting and compliance.
  • Maintain a high level of data accuracy and confidentiality at all times.
  • Verify Medicaid eligibility, submit inquiries, and review case statuses through ePACES.
  • Manage navigation and upkeep of case records within Boelt/BOLT CRM (training will be provided).
  • Handle daily communication, scheduling, and case coordination through Outlook.
  • Collaborate closely with Intake leaders to ensure workflow efficiency.
  • Assist in improving intake procedures and system usage by analyzing trends.
  • Perform other tasks related to the role.

Qualifications & Requirements:

  • Proficient level of English (written and spoken).
  • Excellent interpersonal and communication abilities to collaborate effectively with teams, leadership, providers, and clients.
  • Prior Intake Specialist or Medicaid-focused experience (Intake, Eligibility, Case Management, etc.).
  • Experience with ePACES or similar web-based platforms.
  • Familiarity using Boelt/BOLT CRM or similar healthcare CRM systems.
  • General understanding of different MLTC (Managed Long-Term Care) and MAP (Medicaid Advantage Plus / Medicare Advantage Plan for Dual Eligibles) plans.
  • Knowledge of NYIA (New York Independent Assessor) assessments.
  • Basic knowledge of Medicaid codes and the forms necessary to remove or upgrade a patients Medicaid.
  • Ability to navigate complex intake processes for Home Care beyond simple data entry.
  • Problem-solving skills to manage enrollment challenges and identify solutions efficiently.
  • Comfortable learning new systems and following processes and procedures.
  • Strong understanding of Medicaid eligibility, documentation, and workflows.
  • High attention to detail and ability to manage sensitive information responsibly.
  • Familiarity with U.S. healthcare systems.
  • Strong multitasking and organizational skills.
  • Sound judgment and the capacity to comprehend and execute instructions promptly with minimal supervision.
  • Reliable computer (Windows 10 or newer), two monitors, and stable high-speed internet.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) and Google Workspace (Docs, Sheets), preferred.
