Location: Mishawaka, Indiana
Pay Rate: $48.45 – $50.45 per hour

Position Overview
A growing healthcare organization seeks a licensed Physical Therapist for a 90-day onsite contract in Mishawaka, Indiana. This position delivers evidence-based therapy to a varied patient population focused on restoring function, mobility, and independence.

Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver therapeutic interventions including therapeutic exercise, manual therapy, and modalities
• Monitor patient progress and modify plans of care based on outcomes
• Educate patients and caregivers on home exercise programs and injury prevention
• Collaborate with physicians, occupational therapists, and interdisciplinary teams
• Complete timely and accurate documentation in compliance with payer and regulatory requirements
• Supervise PTAs and support staff as appropriate

Requirements

• Active PT license in good standing (compact or Indiana)
• Minimum 1 year post-licensure clinical PT experience
• BLS certification
• Strong clinical documentation skills
• Must reside 60+ miles from facility (travel assignment requirement)

Preferred Qualifications

• Outpatient orthopedic or acute care experience
• Experience with diverse patient populations
• Familiarity with WebPT or equivalent EHR
