A leading regional healthcare provider known for patient-centric care is seeking Licensed Practical Nurses to join its nursing team. Core Responsibilities

• Administer medications and treatments accurately according to physician orders and facility policy
• Complete resident assessments, monitor changes in condition, and report concerns to the RN supervisor and provider
• Perform wound care, catheter care, injections, and other skilled nursing procedures within LPN scope
• Supervise and support CNAs on the assigned unit to ensure care is delivered safely and on time
• Document assessments, medication administration, and care in the electronic health record
• Participate in care planning, admission, and discharge processes
• Respond to emergencies and initiate appropriate interventions
• Communicate clearly with residents, families, therapy, and the interdisciplinary team
• Follow infection control, fall prevention, and resident rights standards
• Take part in shift reports, audits, and ongoing clinical education
• Mentor newer CNAs and nurses and model professional, resident-centered care
• Maintain medication carts, supplies, and treatment records in compliance with regulations

Required Qualifications

• Active, unrestricted LPN license in the state of practice or an eligible compact license
• Current BLS or CPR certification
• One year or more of long-term care, skilled nursing, or rehab experience preferred; new graduates are welcome to apply
• Working knowledge of electronic health records and medication administration systems
• Strong clinical judgment, organization, and communication skills
• Ability to pass a background check and health screening
• Reliable attendance and flexibility to work some weekends and holidays as scheduled
• Commitment to resident dignity, privacy, and person-centered care

Benefits & Pay
Pay: $35.00 - $45.00 per hour, based on experience and schedule.
