- Monitor service delivery quality and address gaps, complaints, or compliance issues in a timely manner
- Manage caseload assignments and scheduling to ensure adequate coverage and continuity of care
- Maintain accurate documentation and reporting in compliance with state and agency requirements
- Serve as a point of escalation for care coordinators, clients, and family members with complex needs
Must-Haves / Requirements:
- 2+ years of experience in home care, community care coordination, or a related social services or healthcare role
- Familiarity with Illinois community care programs, Medicaid waiver services, or aging services
- Prior supervisory or lead experience overseeing direct care or coordination staff
- Strong organizational and documentation skills with experience in care management platforms
- Valid driver’s license and ability to conduct field visits as needed
Preferred Qualifications:
- Bachelor’s degree in social work, healthcare administration, nursing, or a related field
- Direct experience within the Illinois Community Care Program administered by the Department on Aging
- Bilingual in English and Spanish
Screening Questions:
- Do you have experience working with Illinois community care programs, Medicaid waiver services, or the Department on Aging?
- Have you supervised or managed a team of care coordinators or direct care staff? How many people did you oversee and how did you manage quality and accountability?
- Are you comfortable conducting home visits and field travel as a regular part of your role?
