Key Responsibilities
- Coordinate and oversee the scheduling and delivery of skilled nursing and therapy services for home health patients
- Review and process physician orders, referrals, and authorizations in a timely and accurate manner
- Conduct patient assessments and assist with the development of individualized care plans
- Communicate regularly with field clinicians, physicians, patients, and family members to ensure care continuity
- Monitor patient progress and update care plans to reflect changes in clinical condition
- Ensure all clinical documentation meets regulatory and compliance standards, including Medicare and Medicaid guidelines
- Participate in quality assurance and performance improvement initiatives
- Mentor and support field nursing staff with clinical questions and documentation requirements
- Collaborate with the billing and administrative teams to resolve authorization and documentation issues
- Attend and contribute to case conference meetings and multidisciplinary team discussions
Requirements
- Active Registered Nurse (RN) license in the state of Texas, in good standing
- Minimum of two years of clinical nursing experience, preferably in home health or a related setting
- Strong understanding of Medicare Conditions of Participation and home health regulations
- Proficiency with electronic medical record (EMR) systems
- Excellent organizational, communication, and time management skills
- Ability to manage multiple priorities simultaneously in a deadline-driven environment
- Strong attention to detail and commitment to documentation accuracy
- Bilingual skills in English and Spanish are a plus but not required
Preferred Qualifications
- Experience in a related field is preferred
