Key Responsibilities
- Conduct thorough initial evaluations and develop individualized treatment plans for patients recovering from surgery, injury, stroke, or managing chronic conditions
- Deliver skilled physical therapy services in patients' private residences throughout the Sterling Heights and surrounding communities
- Establish measurable, functional goals in collaboration with patients and their families
- Monitor and document patient progress accurately and in compliance with all regulatory requirements
- Communicate regularly with referring physicians and the broader care team to ensure coordinated, high-quality care
- Educate patients and caregivers on home exercise programs, safety strategies, and fall prevention techniques
- Manage a caseload efficiently while maintaining excellent standards of clinical care and documentation
- Participate in case conferences and contribute to ongoing quality improvement initiatives
Requirements
- Active Michigan Physical Therapist license in good standing
- Graduate degree from an accredited physical therapy program
- Minimum of one year of clinical experience, preferably in home health or a related setting
- Valid driver's license and reliable personal transportation
- Strong working knowledge of Medicare home health regulations and documentation standards
- Demonstrated ability to work independently with excellent time management and organizational skills
- Proficiency with electronic medical records systems
- Outstanding interpersonal and communication skills with a patient-first mindset
Preferred Qualifications
- Experience in a related field is preferred
