Pay Rate: $75,000 to $95,000 per year

Position Overview
A growing healthcare organization is seeking a dedicated and compassionate Physical Therapist to join a dynamic home health team in Sterling Heights, Michigan. Key Responsibilities

• Conduct thorough initial evaluations and develop individualized treatment plans for patients recovering from surgery, injury, stroke, or managing chronic conditions
• Deliver skilled physical therapy services in patients' private residences throughout the Sterling Heights and surrounding communities
• Establish measurable, functional goals in collaboration with patients and their families
• Monitor and document patient progress accurately and in compliance with all regulatory requirements
• Communicate regularly with referring physicians and the broader care team to ensure coordinated, high-quality care
• Educate patients and caregivers on home exercise programs, safety strategies, and fall prevention techniques
• Manage a caseload efficiently while maintaining excellent standards of clinical care and documentation
• Participate in case conferences and contribute to ongoing quality improvement initiatives

Requirements

• Active Michigan Physical Therapist license in good standing
• Graduate degree from an accredited physical therapy program
• Minimum of one year of clinical experience, preferably in home health or a related setting
• Valid driver's license and reliable personal transportation
• Strong working knowledge of Medicare home health regulations and documentation standards
• Demonstrated ability to work independently with excellent time management and organizational skills
• Proficiency with electronic medical records systems
• Outstanding interpersonal and communication skills with a patient-first mindset

Preferred Qualifications

• Experience in a related field is preferred
