Director of Case Management

Job Overview:
We are seeking an experienced Director of Case Management to lead and oversee care coordination, utilization review, and discharge planning services in a respected healthcare facility in Ottumwa, Iowa. This role is critical in ensuring effective patient care transitions, cost efficiency, and adherence to quality care standards. The ideal candidate will be a registered nurse with a strong background in case management and leadership in healthcare operations.

Key Responsibilities:

Direct and manage all aspects of the case management program, including utilization review and discharge planning.
Collaborate with interdisciplinary teams to promote efficient patient flow and care coordination.
Ensure regulatory compliance and alignment with best practices in cost and quality management.
Analyze patient data and trends to drive improvements in care delivery and resource utilization.
Lead and develop the case management team, supporting staff performance and professional growth.

Required Qualifications:

Graduate of an accredited Registered Nursing program.
Current RN license in Iowa or multistate license with eligibility to work in Iowa.
Minimum of 2 years of experience in case management, utilization management, discharge planning, or related areas.

Previous experience in a hospital-based or integrated health system setting.
Strong understanding of care transitions, reimbursement models, and quality improvement initiatives.
