Primary Care Physician

Position Overview

Were seeking a mission-driven Primary Care Physician to provide comprehensive care to a medically complex, primarily elderly population. This role offers the chance to work within an integrated model that blends medical and social care, allowing for a more holistic, proactive approach to primary care. Whether your background is in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics, you'll play a vital role in improving quality of life through continuity, compassion, and coordination.

Required Qualifications

• Medical degree from an accredited institution (MD or DO)

• Board eligibility or board certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics (board certification preferred)

• One to two years of post-residency experience in a primary care setting

• Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants
