Care Management Manager RN

Manager of Care Management (RN or MSW) at our client in Fullerton, CA.

This is a full-time position that will lead and manage the Care Management Team of Registered Nurses and Social Workers.

Under the direction of the Director, Care Management, the Manager, Care Management, is responsible for the overall daily operations and clinical practice of the assigned service areas. This position will oversee Case Managers and Care Management Coordinator support staff to ensure the proper case management of our patients through the continuum of care. The Manager, Care Management, ensure the Case managers to advocate for and assist the patient in the achievement of optimal health, access to care, and appropriately utilizing resources.

Requirements:

• Bachelors degree in nursing or master's degree in social work.
• 3 years of recent experience working as a RN or as a Social Worker in an Ambulatory or Acute Care / Hospital setting
• 1 year of recent experience in leadership, managing people directly and overall operations of running a department in a healthcare setting
• Strong project management and operations experience
• Master's degree in nursing or health administration preferred
• Case Management Certification (CCM or ACM) upon hire
