Job Brief:

We are recruiting for a Home Health Physical Therapist for a leading National 5-Star Home Health Provider in Peoria, AZ. In this vital field role, you will provide direct patient care and manage therapy cases within the community to optimize healthcare outcomes. This position offers a competitive salary, mileage reimbursement, and up to $5,000 signing bonus after 90 days.

Responsibilities

• Provide physical therapy services in patients homes.
• Perform assessments, develop personalized treatment plans, and deliver therapeutic interventions.
• Educate patients and families on home exercise programs and techniques.
• Monitor and document patient progress, adjusting treatment plans as needed.
• Collaborate with healthcare professionals to ensure comprehensive care.
• Design and/or order appropriate adaptive/assistive devices.

Requirements

• License: Active Physical Therapist (PT) license in Arizona with no restrictions
• Experience: At least 1 year of experience supporting in-home patients

Skills:

• Proficient in therapy software and documentation
• Strong organizational and communication skills

Traits:

• Empathetic and detail-oriented
• Capable of working independently in field settings
