Responsibilities

• Deliver physical therapy services in patients homes.
• Conduct assessments, diagnostic tests, skilled procedures, and ongoing evaluations.
• Develop and execute personalized treatment plans tailored to each patients needs.
• Educate patients and their families on exercise programs and rehabilitation techniques.
• Monitor and document patient progress with detailed and accurate reporting.
• Collaborate with multidisciplinary teams to provide comprehensive patient care.
• Design and/or recommend appropriate adaptive and assistive devices for patients.

Requirements

• Certification: Licensed Physical Therapist (PT) with no restrictions in California.
• Experience: Home health experience preferred but not required.

Skills:

• Exceptional interpersonal and communication skills.
• Ability to work both independently and as part of a collaborative team.
• Proficiency in therapy software and documentation tools.

Physical Demands:

• Full range of body motion and ability to lift patients.
• A valid drivers license and reliable transportation.
