Responsibilities:

Provide occupational therapy services in patients homes.

Perform assessments, diagnostic tests, skilled procedures, and ongoing evaluations.

Develop and implement personalized treatment plans.

Educate patients and families on home exercise programs and techniques.

Monitor and document patient progress.

Collaborate with healthcare professionals to ensure comprehensive care.

Design and/or order appropriate adaptive/assistive devices.

Requirements:

Certification: Licensed Occupational Therapist (OT) with no restrictions.

Experience: Home health experience preferred.

Skills:

Strong interpersonal and communication skills.

Ability to work independently and as part of a team.

Proficiency in therapy software and documentation.

Physical Demands:

Full range of body motion and ability to lift patients.

Requires a current drivers license and ability to drive.
