We're in search of a dedicated PRN RN Case Manager to join a Home Health team. This role, based in the heart of Anthem, AZ, offers a unique opportunity to provide critical care within the comfort of patients' homes, blending flexibility with the satisfaction of impactful work.
Your Role:
As a PRN RN Case Manager, you'll be instrumental in assessing patient needs, planning and coordinating care, and ensuring the delivery of high-quality home health services. Working 10-30 hours a week, you'll play a key role in a team dedicated to making a positive impact on the lives of those we serve.

- Compensation: Competitive pay ranging from $45.00 to $120.00 per hour. Based on a median hourly rate, working as a PRN RN Case Manager can potentially earn you:
- At 10 hours a week: Approximately $42,900 per year.
- At 30 hours a week: Up to $128,700 per year.

Qualifications:
- Graduate from an approved school of professional nursing, Baccalaureate degree favored.
- Current RN license as required by the state of Arizona.
- Ability to supervise and direct members of the client care team, with demonstrated organizational and time management skills.
- Strong written and verbal communication skills, good interpersonal skills, and the ability to deal tactfully with clients and the community.
- Licensed driver with an insured automobile in accordance with state and Agency requirements. Good driving record and reliable transportation are essential.
- Current CPR certification.
