Care Manager Requirements
- Bachelors degree in a related field preferred (required for detox level of care); high school diploma required at minimum
- Minimum of one year of experience working in the substance use disorder field
- Knowledge of addiction, mental health, treatment resources, and 12-Step recovery
- Familiarity with discharge planning, aftercare coordination, and case management
- CPR certification preferred
- CRSS, CAC, CAP, LCSW, LMHC, or Registered Intern (preferred, willingness to obtain a plus)
Care Manager Responsibilities
- Coordinate discharge planning and assist clients with transitions to aftercare programs
- Communicate with outside providers, referents, and family members (with proper releases in place)
- Facilitate individual case management sessions and maintain ongoing communication with clients
- Participate in intervention strategies for clients at risk of leaving treatment Against Medical Advice (AMA)
- Monitor client participation, attendance, and adherence to the daily treatment schedule
- Assist with clinical documentation, including assessments and treatment planning, as appropriate
- Facilitate weekly clinical groups and participate in recreational and weekend activities as needed
- Maintain accurate and timely documentation in accordance with company standards
- Coordinate and assist with client transportation as needed
- Assist clients with daily living essentials and ancillary financial accounts
- Monitor and support agency drug screening processes under supervision
- Uphold patient privacy and confidentiality in accordance with HIPAA and 42 CFR regulations
- Report concerns about client privacy, safety, or quality of care to appropriate leadership without fear of retaliation
- Support and enforce facility policies, procedures, and program expectations
- Perform other duties as assigned
Pay: $40,000-60,000
Schedule: Full-time
