Care Manager Requirements

  • Bachelors degree in a related field preferred (required for detox level of care); high school diploma required at minimum
  • Minimum of one year of experience working in the substance use disorder field
  • Knowledge of addiction, mental health, treatment resources, and 12-Step recovery
  • Familiarity with discharge planning, aftercare coordination, and case management
  • CPR certification preferred
  • CRSS, CAC, CAP, LCSW, LMHC, or Registered Intern (preferred, willingness to obtain a plus)

Care Manager Responsibilities

  • Coordinate discharge planning and assist clients with transitions to aftercare programs
  • Communicate with outside providers, referents, and family members (with proper releases in place)
  • Facilitate individual case management sessions and maintain ongoing communication with clients
  • Participate in intervention strategies for clients at risk of leaving treatment Against Medical Advice (AMA)
  • Monitor client participation, attendance, and adherence to the daily treatment schedule
  • Assist with clinical documentation, including assessments and treatment planning, as appropriate
  • Facilitate weekly clinical groups and participate in recreational and weekend activities as needed
  • Maintain accurate and timely documentation in accordance with company standards
  • Coordinate and assist with client transportation as needed
  • Assist clients with daily living essentials and ancillary financial accounts
  • Monitor and support agency drug screening processes under supervision
  • Uphold patient privacy and confidentiality in accordance with HIPAA and 42 CFR regulations
  • Report concerns about client privacy, safety, or quality of care to appropriate leadership without fear of retaliation
  • Support and enforce facility policies, procedures, and program expectations
  • Perform other duties as assigned

Pay: $40,000-60,000
Schedule: Full-time
