Guardian Recovery Network is a nationwide provider specializing in substance use disorder primary treatment. The Plymouth House is a featured location in Plymouth, NH. We are a residential and partial hospitalization level of care. We are looking for a Family Nurse Practitioner.

We are a leading provider of mental health and substance abuse treatment services, dedicated to transforming lives and communities through compassionate care and evidence-based practices. We are currently seeking a skilled Family Nurse Practitioner (FNP) to join our team on a per diem basis, supporting patients in detox and residential substance abuse treatment.

As a Family Nurse Practitioner (FNP) in our detox and residential program, you will play a vital role in supporting patients through the early stages of recovery. You will collaborate with a multidisciplinary team to manage withdrawal symptoms, monitor patient health, and provide comprehensive medical care in a structured and supportive environment.

Requirements

  • Masters or Doctoral degree in Nursing with Family Nurse Practitioner certification
  • Active and unencumbered FNP license in the state of NH
  • Minimum of 2 years of experience in detox, substance abuse treatment, or behavioral health settings preferred
  • Knowledge of withdrawal protocols, detox treatment procedures, and medication management
  • Strong interpersonal and communication skills
  • CPR/BLS certification required
  • Commitment to promoting a culture of diversity, equity, and inclusion

Responsibilities

  • Conduct comprehensive health assessments, including evaluations related to substance withdrawal and detoxification
  • Develop and manage individualized treatment plans to support patients in detox and residential settings
  • Prescribe and monitor medications for withdrawal management and symptom control
  • Collaborate with multidisciplinary team members, including counselors and case managers, to ensure continuity of care
  • Educate patients on recovery options, self-care during withdrawal, and long-term wellness strategies
  • Monitor patient progress and adjust treatment plans as necessary, documenting all interactions thoroughly
  • Provide crisis support and guidance to patients, helping them navigate the early stages of recovery
  • Ensure compliance with state and federal regulations, including HIPAA, to protect patient confidentiality
  • Participate in team meetings, training sessions, and quality improvement initiatives
  • Perform other duties as assigned

Pay: $75.00 - $95.00 per hour
Schedule: Per diem
