As a Client Care Coordinator, youll play a crucial role in helping clients navigate their treatment journey. You will provide personalized case management and discharge planning to ensure a smooth, supportive experience from intake through discharge. Requirements

  • Bachelors degree in Social Work, Psychology, Counseling, or a related field (Masters preferred)
  • Experience in case management, discharge planning, or behavioral health
  • Strong communication and interpersonal skills
  • Knowledge of community resources and referral processes
  • Ability to manage multiple priorities in a fast-paced environment
  • Compassionate, patient-centered approach

Responsibilities

  • Case Management: Assess clients' needs, create individualized care plans, and coordinate services throughout their stay.
  • Discharge Planning: Develop comprehensive discharge plans, including identifying resources and support for ongoing recovery.
  • Client Advocacy: Ensure clients receive the care and support they need, acting as a liaison between the client, treatment team, and external resources.
  • Documentation: Maintain accurate and timely records of client interactions, progress notes, and discharge plans.
  • Resource Coordination: Connect clients with external services such as housing, job placement, and community support groups.
  • Collaboration: Work closely with clinical and administrative teams to ensure seamless client care.

Job Type: Full Time

Schedule: M-F; 9-5pm with on call duties

Location: Plano, TX

Pay: $55,000 - $65,000 annually
