Were seeking a Nurse Practitioner or Physician Assistant to serve as a Rounding Provider in one or more senior living communities. This position involves direct patient care, coordination with interdisciplinary teams, and proactive engagement in evaluation and management (E/M) visits. You will operate with a degree of autonomy while benefiting from the support of a dedicated telemedicine team and internal infrastructure.
This is a daytime role, typically from 7:00 AM to 1:00 PM, with flexibility to manage your own workflow and prioritize patient care efficiently.

Requirements
- Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Missouri
- Current DEA license
- Experience in geriatrics, internal medicine, or post-acute care preferred
- Familiarity with SNF workflows and interdisciplinary coordination
- Ability to work autonomously and manage patient caseload efficiently
- Comfortable using EHR systems (PCC/Matrix and ChartEasy preferred)
- Excellent communication and documentation skills
- Must meet all Michigan-specific requirements for advanced practice providers in long-term care, including adherence to collaborative agreement protocols if applicable
Responsibilities
- Provide day-to-day medical management of residents in assigned skilled nursing facilities
- Perform Evaluation & Management (E/M) visits for:
- New admissions (on a 2-2-2 schedule)
- Chronic residents (e.g., medication reviews, dose reductions)
- Acute concerns (falls, behavioral changes, weight loss, etc.)
- Review and act on pharmacy recommendations
- Order durable medical equipment (DME) and manage discharge preparations
- Collaborate with physical, occupational, and speech therapists, dietitians, respiratory therapists, and facility staff
- Document visits thoroughly and submit within 48 hours
- Communicate with the telemedicine team regarding high-risk patients or after-hours coverage needs
- Review labs, hospital discharge summaries, and communicate updates in Secure Messaging and facility EHR
Location: Oak Grove, MO
Pay: $100,000 - $150,000 per year with RVUs
