About the Role

We are seeking an experienced and compassionate Nurse Practitioner to join our interdisciplinary healthcare team in a skilled nursing and assisted living environment. This role focuses on providing high-quality, resident-centered medical care, chronic disease management, and collaborative treatment planning.

Key Responsibilities

  • Conduct comprehensive physical exams, assessments, and health histories for new admissions and ongoing residents
  • Diagnose and manage acute and chronic conditions such as diabetes, hypertension, arthritis, and dementia
  • Order, interpret, and act on diagnostic tests including blood work, imaging, and routine screenings
  • Develop and update individualized care plans; document in EHR and ensure compliance with regulatory and facility standards
  • Provide patient and family education on disease management, medication regimens, and preventative care
  • Collaborate with physicians, nursing staff, therapists, and support staff; participate in family meetings and interdisciplinary rounds
  • Assist with discharge planning and transitions to other levels of care (e.g., home health, hospital, hospice)
  • Address urgent changes in resident status and initiate appropriate interventions
  • Serve as on-call provider, responding to staff inquiries and emergent issues
  • Mentor and support nursing staff in clinical best practices; contribute to quality improvement initiatives

Required Qualifications

  • Masters (MSN) or Doctorate (DNP) in Nursing with Nurse Practitioner certification
  • Active Michigan NP licensure and DEA certification
  • Minimum 2 years of clinical NP experience, preferably in geriatrics, long-term care, or primary care
  • Valid BLS/ACLS certification
  • Strong skills in patient assessment, diagnosis, and medication management
  • Excellent communication, empathy, and ability to work well within a multidisciplinary care environment

Preferred Experience

  • Skilled Nursing Facility (SNF), assisted living, memory care, or hospice background
  • Proficient with EHR systems
  • Familiarity with regulatory documentation and compliance standards
