Job Summary

We are seeking a Certified Alcohol and Substance Abuse Counselor (CASAC) or equivalent credential to join our dynamic team as a Case Manager. This role is responsible for coordinating care, advocating for clients, and ensuring a seamless transition through different levels of treatment. The ideal candidate has experience in substance use disorder treatment, strong clinical documentation skills, and a passion for helping individuals achieve long-term recovery.

Responsibilities

  • Case Management & Client Advocacy:
  • Develop, implement, and monitor individualized treatment plans.
  • Act as the primary point of contact for clients, families, and referral sources.
  • Collaborate with clinical staff to ensure continuity of care and treatment effectiveness.
  • Assist clients in accessing resources such as housing, employment support, and aftercare programs.
  • Clinical Documentation & Compliance:
  • Maintain accurate and timely documentation in compliance with state, federal, and accreditation standards.
  • Complete assessments, progress notes, and discharge planning reports.
  • Ensure all treatment plans align with evidence-based practices and facility policies.
  • Collaboration & Communication:
  • Work closely with therapists, medical staff, and community organizations.
  • Participate in multidisciplinary team meetings to review client progress.
  • Facilitate psychoeducational and support groups as needed.

Qualifications

  • Active CASAC certification or equivalent licensure (e.g., CADC, LAADC, or other state-recognized substance use counseling credential).
  • Minimum 2 years of experience in substance use disorder treatment or behavioral health settings.
  • Strong knowledge of case management principles, relapse prevention strategies, and community resources.
  • Experience working in residential, PHP, IOP, or outpatient settings preferred.
  • Proficiency in electronic health records (EHR) and clinical documentation.
  • Ability to work in a fast-paced, team-oriented environment while maintaining professionalism and compassion.
