Job Summary
We are seeking a Certified Alcohol and Substance Abuse Counselor (CASAC) or equivalent credential to join our dynamic team as a Case Manager. This role is responsible for coordinating care, advocating for clients, and ensuring a seamless transition through different levels of treatment. The ideal candidate has experience in substance use disorder treatment, strong clinical documentation skills, and a passion for helping individuals achieve long-term recovery.
Responsibilities
- Case Management & Client Advocacy:
- Develop, implement, and monitor individualized treatment plans.
- Act as the primary point of contact for clients, families, and referral sources.
- Collaborate with clinical staff to ensure continuity of care and treatment effectiveness.
- Assist clients in accessing resources such as housing, employment support, and aftercare programs.
- Clinical Documentation & Compliance:
- Maintain accurate and timely documentation in compliance with state, federal, and accreditation standards.
- Complete assessments, progress notes, and discharge planning reports.
- Ensure all treatment plans align with evidence-based practices and facility policies.
- Collaboration & Communication:
- Work closely with therapists, medical staff, and community organizations.
- Participate in multidisciplinary team meetings to review client progress.
- Facilitate psychoeducational and support groups as needed.
Qualifications
- Active CASAC certification or equivalent licensure (e.g., CADC, LAADC, or other state-recognized substance use counseling credential).
- Minimum 2 years of experience in substance use disorder treatment or behavioral health settings.
- Strong knowledge of case management principles, relapse prevention strategies, and community resources.
- Experience working in residential, PHP, IOP, or outpatient settings preferred.
- Proficiency in electronic health records (EHR) and clinical documentation.
- Ability to work in a fast-paced, team-oriented environment while maintaining professionalism and compassion.
