Home Health Social Worker

**MSW, SSW, or LCSW Required**

Location: Salt Lake Valley, UT

Pay: $30-35/hour

Job Overview:

We are seeking a compassionate and dedicated Social Worker to provide support to the geriatric community through Home Health. The ideal candidate will have a strong background in providing end of life support, offering community resources, and discussing care goals with patients. This position will be full-time in the Salt Lake City area. You are paid for drive time and mileage!

Responsibilities:

• Communicate effectively with the internal team and those offering patient support
• Organize schedule in an efficient manner to travel to assigned visits
• Provide helpful resources and discuss care goals with elderly patients
• Navigate end of life issues that require support and attention
• Offer crisis management support during difficult situations

Qualifications:

• MSW required
• Valid driver's license
• Experience with geriatric community preferred, not required
