Sigma Systems Inc. is seeking a Community Housing Coordinator on behalf of our client. This position offers the flexibility of remote work combined with meaningful community engagement, supporting members with preventive health, care coordination, and health-related social needs.

Position Overview: Community Housing Coordinator
The Community Housing Coordinator will conduct outreach and engagement activities focused on preventive health, care gaps, maternity, care transitions, health-related social needs (SDOH), and chronic condition initiatives. The coordinator will work directly with members, healthcare providers, community organizations, and human services agencies to connect individuals with appropriate healthcare and community resources.
This role requires strong communication, cultural competency, empathy, and the ability to build trusted relationships with at-risk populations.
Key Responsibilities
Member Outreach and Support
- Initiate outreach efforts both face-to-face and telephonically related to targeted preventive health, care gaps, maternity, care transitions, health-related social needs, and chronic condition initiatives.
- Educate members on the importance of targeted preventive health services.
- Assess opportunities for care management intervention and make appropriate referrals, including referrals to community resources.
- Conduct Social Determinants of Health (SDOH) assessments as needed.
- Assist members with scheduling appointments, addressing social health barriers, and answering questions.
- Contact members, providers, and community agencies to coordinate access to preventive health services and assistance with health-related social needs.
Community Engagement and Outreach
- Develop strong, supportive relationships with members, community-based organizations, and provider partners.
- Empower members to take an active role in their health and wellness.
- Collaborate with human services providers, including Head Start programs, WIC, community centers, homeless shelters, and other community organizations.
- Assist with identifying and reaching members who may need additional services or support.
- Represent the company at community events, including events sponsored by community partners.
Education and Communication
- Contribute to the development and preparation of educational materials for members and providers.
- Provide summaries and outcomes of outreach efforts and community education activities to management and peers.
- Provide members with accurate and current information regarding the organization's lines of business and available benefits.
Program Development and Continuous Improvement
- Participate in the development of effective, measurable, and innovative programs focused on reaching at-risk members.
- Identify opportunities to improve outreach, education, and member support services.
- Perform other duties as assigned or requested.
Qualifications
Education & Experience
- Minimum 1 year of Community Health Worker experience or related experience.
- Lived experience within the community served is preferred.
- Experience with Medicaid regulations.
- Experience in customer service/member services within the healthcare sector.
- Experience working with at-risk populations.
- Bilingual in English and Spanish required.
Licenses or Certifications
- Certified Community Health Worker (CCHW) certification through the PA Certification Board is required within 12 months of the start date.
Skills
- Knowledge of customer service principles, including customer needs assessment and meeting quality standards for services.
- Ability to interact effectively with peers, supervisors, members, and customers and work collaboratively as part of a team.
- Strong problem-solving skills with the ability to work independently, creatively, proactively, and assertively.
- Ability to manage multiple tasks simultaneously and respond promptly to member needs and concerns.
- Excellent written and verbal communication skills.
- Cultural competency and sensitivity when working with Medicaid populations.
- Demonstrated expertise and sensitivity when working with members with limited English proficiency.
- Understanding of healthcare-related barriers, including transportation, childcare, limited knowledge of preventive health services, distrust of the healthcare system, and other personal, social, and financial challenges.
- Demonstrated patience, empathy, and professionalism when interacting with members and internal/external customers.
Work Schedule & Conditions
- Remote position with 50–75% local travel required; travel is reimbursable.
- Must be bilingual in English and Spanish.
- Must reside within one of the following Pennsylvania counties:
- Adams
- Lancaster
- Lebanon
- York
- Regular business hours, Monday through Friday.
- Occasional evening or weekend availability may be required for community events.
