Job Title: Remote Case Manager

Hourly Pay: $23 - $38/hour

Job Responsibilities


  • Conduct initial client assessments to determine needs and create personalized case management plans
  • Collaborate with healthcare providers, social services, and community organizations to coordinate care for clients
  • Monitor clients' progress and adjust care plans as needed to achieve their goals
  • Provide ongoing support and advocacy for clients to ensure they have access to required services
  • Ensure clients understand and utilize available resources such as financial assistance, healthcare, housing, and education
  • Maintain accurate, confidential client records and documentation

  • Facilitate communication between clients, family members, and service providers
  • Provide crisis intervention as necessary, offering immediate support during critical situations

Qualifications


  • Bachelor’s degree in Social Work, Psychology, or a related field (MSW preferred)
  • 2+ years of experience in case management or social services

  • Strong understanding of social service resources, healthcare, and community programs
  • Excellent communication, organizational, and problem-solving skills

  • Ability to work independently and handle a varied caseload

  • Strong interpersonal skills, with an ability to work with diverse populations
  • Valid state licensure or certification as required
