Qualifications Required:

- Valid Drug Enforcement Administration (DEA) certification/license

- Home Health Care
- Hospice
- Managed Care (2+ years)

Overview:
The company supports patients suffering from Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) by forming deep-rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease.
Position Summary:
Nurse Practitioners are responsible for delivering personalized compassionate medical care to our patients. Primary duties include patient health assessments, conducting physicals, prescribing medications, maintaining accurate records, scheduling appointments, administering follow-up appointments, and working collaboratively with the care management team to ensure patients receive quality, evidence-based care. This position involves conducting in-home visits in the state of residence and occasional training/meetings at headquarters outside of the home state.
Roles and Responsibilities:
- Deliver evidence-based, timely care that reduces hospitalizations, maximizes quality of life, and prioritizes patient health and satisfaction.
- Conduct comprehensive annual wellness exams.
- Prescribe medications, order tests, and collaborate with healthcare professionals.
- Collaborate with care management RNs and social workers for patient care planning.
- Practice autonomously with the ability to work without direct in-person supervision.
- Heavy daily travel within a one-hour radius from the main location, including 3-4 days on the road visiting patients and 1-2 days of telehealth.
Position Requirements:
- Active and unrestricted Registered Nurse and Nurse Practitioner license in the state.
- Board-certified by ANCC or AANP.
- Current and unrestricted DEA certificate.
- Ability to work independently and practice autonomously.
- Heavy daily travel, access to transportation, a valid driver's license, and car insurance.
- Proficiency with medical instruments and equipment.
- Knowledge of federal and state regulations, including HIPAA and OSHA.
- 2+ years of NP experience.
- Bilingual in Spanish preferred.
- Experience in kidney disease and related co-morbidities (HTN, Diabetes, CHF) preferred.
- Experience in end-of-life discussions, palliative care, and hospice preferred.
- Experience practicing in managed care, value-based context required.
- Experience conducting annual wellness visits, comprehensive visits, and appropriately charting HCCs.
