We are seeking a Care Management Services Director to lead patient-centered initiatives and ensure seamless transitions across the continuum of care. Key Responsibilities:

• Oversee case management, social work, utilization management, and post-acute services coordination.
• Develop and enhance care facilitation programs to ensure smooth patient transitions.

• Collaborate with healthcare providers to enhance care coordination and resource utilization.
• Ensure compliance with healthcare regulations and best practices.

Requirements:

• Licensure: Active RN license.
• Experience: At least five years of experience in case management within a healthcare setting.
• Skills: Strong leadership, problem-solving, and communication abilities to engage with patients, families, and healthcare teams.

Preferred:

• Education: Advanced degree in healthcare or related field.
• Leadership Experience: Three to five years in a supervisory role.
• Training: Formal case management training.
