Overview Our client is seeking a compassionate and dedicated Home Health Licensed Practical Nurse (LPN) to join their exceptional care team in New Bloomfield, PA. The ideal candidate will bring strong clinical skills, reliable professionalism, and a genuine commitment to supporting patients in their homes. This role requires an Associate degree or higher, a valid Pennsylvania LPN license, and experience working with EMR systems. Candidates should be comfortable managing their schedules independently and meeting critical documentation deadlines.

Key Responsibilities

• Collaborate with the care team to manage and deliver individualized patient care plans
• Provide direct support to patients within the New Bloomfield, PA area
• Complete charting and documentation accurately and on time
• Self-manage daily schedule and travel efficiently
• Utilize tablets, mobile devices, and EMR software confidently (including TruChart EHR and other EMR systems)

Required Qualifications

• Associate degree or higher
• 2 + years LPN experience 
• Valid Licensed Practical Nurse (LPN) license in Pennsylvania (PA)
• 1+ years of experience in ANY of the following:
  • Electronic Medical Records (EMR)
  • Other EMR systems
  • TruChart EHR
• ANY of the following valid certifications:
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • American Heart Association Basic Life Support (AHA BLS)

Schedule & Shift Details

Travel Requirements

• Occasional travel may be required within the local service area.
