Registered Nurse Case Manager – Direct Hire
Location: Illinois (local travel across multiple counties required) Schedule: Monday–Friday, 8:00 AM–5:00 PM (start/end times vary daily) On‑Call Requirements:
- One (1) 24‑hour weekend day shift per month
- Two (2) weekday on‑call shifts per month (5 PM–8 AM)
- Two (2) holidays per year (1 summer, 1 winter)
- Three floating holidays
- Additional stipends and compensation provided for on‑call coverage
Overview
This is an opportunity for a dedicated Registered Nurse Case Manager to join a compassionate hospice team and make a meaningful impact in the lives of patients and families. You will play a vital role in coordinating care, supporting interdisciplinary teams, and ensuring patients receive exceptional end‑of‑life care. This position is ideal for an RN who thrives in leadership, field‑based care, and patient advocacy.
Key Responsibilities
- Manage and oversee patient care within hospice settings
- Conduct comprehensive assessments and develop individualized care plans
- Coordinate care with physicians, social workers, and interdisciplinary teams
- Provide leadership, guidance, and support to nursing staff
- Ensure compliance with hospice standards, policies, and regulatory requirements
- Maintain accurate documentation and timely reporting
- Travel locally to visit patients across assigned counties
Required Qualifications
Education:
- High School Diploma or GED
Experience:
- 1+ year Hospice experience
- 1+ year Management or Leadership experience
- 1+ year experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health
Licensure:
- Valid Registered Nurse (RN) license in the state of Illinois (IL)
Certifications (ALL required):
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Schedule & Shift Details
- Monday–Friday, 8:00 AM–5:00 PM (flexible daily start/end times)
- On‑Call:
- One 24‑hour weekend day per month
- Two weekday on‑call shifts (5 PM–8 AM)
- Two holidays per year (1 summer, 1 winter)
- Three floating holidays
- Dedicated after‑hours RN team available nightly (3 RNs per night)
Travel Requirements
- Local travel required across multiple counties
- Must be comfortable working in field‑based hospice environments
