The Hospice RN will manage and deliver individualized patient care plans, support patients and families, and consistently meet critical charting deadlines. The ideal candidate is an experienced hospice nurse who excels in autonomy, time management, and patient-centered care.
Key Responsibilities
Provide management and delivery of individualized hospice patient care plans Conduct comprehensive assessments and ongoing evaluations of patient needs Consistently meet critical deadlines for charting and documentation Collaborate with interdisciplinary hospice team members to ensure coordinated care Educate patients and families on care plans, symptom management, and available resources Self-manage time, schedule, and daily patient visits Maintain compliance with all regulatory, clinical, and organizational standards
Schedule & Shift Details
Full-time weekday schedule On-call rotation: Every 3rd week Occasional local travel to patient homes and facilities
Required Qualifications
Associate Degree or higher Valid Registered Nurse (RN) license in Pennsylvania (PA) 2+ years of Hospice experience 1+ years of experience in ANY of the following: – Electronic Medical Record (EMR) – Other EMR systems – TruChart EHR Strong ability to self-manage time, schedule, and documentation Compassionate approach to end-of-life care Excellent communication and patient advocacy skills
