Registered Nurse Case Manager – Hospice Location: Ambridge, PA Schedule: 8 AM – 5 PM, Monday–Friday | 6 units daily / 30 units weekly On‑Call: 1 weekend per month + 1 evening per week Territory: Ambridge and approximately 15–25 miles surrounding the community Travel: Local travel across multiple counties required

This role offers a meaningful opportunity for a compassionate Registered Nurse Case Manager to support patients and families navigating hospice care. You will serve as a clinical leader, coordinating care plans, guiding nursing staff, and ensuring patients receive exceptional, dignified end‑of‑life care. The ideal candidate thrives in a field‑based environment, excels in communication, and brings both hospice experience and leadership capability.

Key Responsibilities

  • Manage and oversee patient care within hospice settings, ensuring high‑quality, patient‑centered services
  • Develop, implement, and evaluate individualized care plans
  • Coordinate with interdisciplinary healthcare teams, including physicians, social workers, and support staff
  • Provide leadership, mentorship, and guidance to nursing staff
  • Maintain accurate, timely clinical documentation
  • Communicate effectively with patients, families, and caregivers
  • Participate in on‑call rotation (one evening weekly + one weekend monthly)
  • Travel locally to patient homes and facilities across multiple counties

Required Qualifications

  • Education: High School Diploma or GED
  • Experience:

    • 1+ years of Hospice experience
    • 1+ years of Management or leadership experience
    • 1+ years of experience in ANY of the following:

      • Home Health Care
      • Epic Home Health
      • Home Health
  • Licensure:

    • Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
  • Certifications (ALL required):

    • American Heart Association Basic Life Support (AHA BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • Other Requirements:

    • Reliable transportation and ability to travel locally
    • Strong time‑management and self‑organization skills
    • Ability to work independently in field‑based environments
