Nurse Practitioner (NP/FNP)

32‑Hour Contract | Chicago, IL | Family Medicine

Overview

Our client is seeking an experienced Nurse Practitioner (NP/FNP) to provide family medicine care to a diverse patient population in Chicago, IL. This contract role offers flexible scheduling and the opportunity to care for patients aged 2 and older. Spanish proficiency is highly valued, and candidates with DOT certification are a plus.

Required Qualifications

• 1+ years of experience in ANY of the following: Internal Medicine Family Practice Med‑Ped
• Valid license/certification in ANY of the following: Nurse Practitioner (NP) Family Nurse Practitioner (FNP)
• Experience with EMR systems (including TruChart EHR or other EMRs)

Preferred Qualifications

• Full proficiency in speaking, reading, and writing Spanish
• DOT certification (preferred but not required)

Key Responsibilities

• Provide family medicine care to patients aged 2 and older
• Deliver clinical services during scheduled shifts
• Conduct assessments, develop treatment plans, and support patient wellness
• Maintain accurate and timely documentation in EMR systems
• Collaborate with clinic staff to ensure high‑quality patient care

Schedule & Shift Details

32 hours per week

• Monday, Wednesday, Friday: 8:30 AM – 5:00 PM
• Thursday: 10:00 AM – 7:00 PM
• Tuesday: 8:30 AM – 1:30 PM OR
• Saturday: 7:00 AM – 12:00 PM
