Nurse Practitioner – Telemedicine Care

Location: Virtual Care (Serving New Mexico) Schedule: Full‑Time

Overview

Candidates may reside anywhere in the United States as long as they hold an active New Mexico NP license and are fully bilingual in Spanish. This role offers an experienced Nurse Practitioner the opportunity to deliver meaningful, high‑impact care to medically complex patients primarily through virtual care . You will join a collaborative interdisciplinary team focused on prevention, early intervention, and improved patient outcomes . The position emphasizes autonomy, care coordination, and innovative approaches to home‑centered healthcare across New Mexico .

Required Qualifications

• Master's degree or higher
• 3+ years of experience in telemedicine care
• Full proficiency in speaking, reading, and writing Spanish
• Ability to manage medically complex patient populations
• Strong communication and care‑coordination skills

Key Responsibilities

• Provide clinical care and ongoing management through telehealth and remote care technologies
• Manage medically complex Medicaid and Dual Eligible patient populations
• Collaborate with physicians and interdisciplinary team members to develop and execute individualized care plans
• Work closely with patients, families, caregivers, primary care providers, specialists, and health plan partners
• Identify changes in patient health and intervene early to prevent unnecessary ED visits and hospitalizations
• Support patients through hospital admissions, discharges, and transitions back home
• Participate in advance care planning and goals‑of‑care discussions
• Participate in a shared clinical triage and support model
• Support quality initiatives including accurate documentation for HCC coding, risk adjustment, STAR/HEDIS measures, and closure of quality gaps
• Contribute to clinical best practices and help shape a growing, innovative care model
