WellSpace Health is looking for a dedicated and compassionate Physician Assistant or Nurse Practitioner to join our Primary Care team in Sacramento. 🩺 What Youll Do

• Provide comprehensive primary care to patients of all ages, from infants to seniors.

• Diagnose and treat acute and chronic conditions.

• Work collaboratively in a team-based, patient-centered care model.

• Educate patients and families on health promotion, chronic disease management, and preventive care.

• Utilize EMR and meet quality and productivity benchmarks aligned with FQHC best practices.

What You Bring

• Licensed NP or PA in California with DEA certification and BLS.

• Board Certification (SP NP, 103 NP, or 104 NP as applicable).

• At least 1 year of post-graduate experience in Primary Care.

• Comfortable working with diverse populations and underserved communities.

• Passion for community health and commitment to continuous improvement.
