Hospice Social Worker (MSW)

This role provides professional social work services within a hospice setting, supporting patients and families through critical stages of care.

Responsibilities

• Conduct psychosocial assessments and assist with discharge planning
• Provide supportive counseling and crisis intervention
• Facilitate therapeutic, support, and education groups
• Assist patients and families in developing coping strategies related to illness and life changes
• Coordinate care and collaborate with interdisciplinary teams
• Participate in community outreach and professional initiatives

Requirements

• Master's degree in Social Work (MSW) required
• Active Ohio license (LSW, LISW, LPC, or LPCC)
• 1+ years of experience preferred
• Strong communication and interpersonal skills
• Schedule: Day Shift (with weekend rotation)
