Hospice Social Worker (MSW)

This role provides professional social work services within a hospice setting, supporting patients and families through critical stages of care.
Responsibilities
- Conduct psychosocial assessments and assist with discharge planning
- Provide supportive counseling and crisis intervention
- Facilitate therapeutic, support, and education groups
- Assist patients and families in developing coping strategies related to illness and life changes
- Coordinate care and collaborate with interdisciplinary teams
- Participate in community outreach and professional initiatives
Requirements
- Master's degree in Social Work (MSW) required
- Active Ohio license (LSW, LISW, LPC, or LPCC)
- 1+ years of experience preferred
- Strong communication and interpersonal skills
- Schedule: Day Shift (with weekend rotation)
