I hereby certify that all information provided in this Application Form is true and complete to the best of my knowledge.  I understand PSO (Manila) Limited Philippine Branch Office may investigate all statements made in this Application and that any false or misleading information provided can result in a decision not to hire me, immediate discharge if hired, and/or civil or criminal penalties as appropriate.

I acknowledge that I have specific rights with respect to my personal data as provided by law, and I am responsible for ensuring that the personal data is correct and true.
