Manager – Interprofessional Primary Care
1 Full-Time, Permanent (35 hours per week)
Reporting To: Director – Community Health Teams
Purpose:
Bring your passion for community-based health care and program development. The Manager of Interprofessional Primary Care works collaboratively with internal stakeholders and partner agencies to provide comprehensive, coordinated services within the community, encompassing primary care, hospice palliative care, health promotion, and sexual assault and domestic violence services.
This role supports integrated, person- and family-centred care through interprofessional team leadership and program development within a diverse and dynamic community health environment.
Key Areas of Responsibility
Leadership and Interprofessional Care:
- Understand, interpret, and implement organizational mission, vision, and values
- Champion person- and family-centred care through integrated primary care practices
- Lead and support change initiatives aligned with strategic priorities
- Provide leadership to interprofessional teams delivering evidence-based care focused on health promotion and illness prevention
- Support expansion of primary care services, including operational readiness at partner sites
- Contribute to program evaluation activities in collaboration with leadership
- Identify opportunities for new program development, service improvement, and operational efficiency
Team Development and Orientation:
- Support professional development plans for team members
- Ensure compliance with relevant legislation and professional standards
- Provide supervision, mentorship, orientation, and teaching to team members
- Participate in interprofessional meetings, task forces, and projects
Operational Leadership:
- Support implementation of primary care work plans
- Assist in accreditation standards and compliance
- Maintain professional competence
- Oversee staff scheduling and on-call coverage
- Improve workflows to enhance access and efficiency
- Support resolution of client complaints as required
Budget and Planning:
- Monitor budgets, including variance analysis and forecasting
- Support annual planning aligned with strategic priorities
- Identify risks and develop mitigation strategies
Qualifications
Education:
- Undergraduate degree in Health Sciences or related field
- Master's degree in a health-related discipline preferred
Experience:
- 3–5 years of experience in interprofessional health care leadership
- Experience supervising staff, students, and volunteers
- Experience in workforce planning and staffing models
- Community health experience considered an asset
Skills and Attributes:
- Strong clinical leadership and critical thinking skills
- Excellent communication and interpersonal abilities
- Proven ability to build collaborative, multidisciplinary teams
- Knowledge of integrated care and social determinants of health
- Proficiency with computers and EMRs
- Ability to work across multiple sites
- Valid Ontario driver's licence and access to a vehicle required
Working Conditions:
- Salary range: $85,149 – $115,126 annually
- Flexible hours required, including evenings and weekends
- Travel between sites required
