About the Opportunity
We are seeking an experienced and dedicated Nurse Practitioner (NP) to join a collaborative and community-based Youth Wellness Hubs Ontario (YWHO) team in Scarborough. This role has a strong mental health focus, supporting youth and young adults aged 1225 through accessible, trauma-informed, and integrated primary care.
Working within a dynamic interprofessional teamincluding counsellors, psychotherapists, peer support workers, and service navigatorsthe NP will play a vital role in bridging physical and mental health care for youth in a low-barrier, client-centered environment.
Position Details
- Employment Type: Part-Time, Permanent (21 hours/week)
- Location: Hybrid onsite with some virtual service options
- Hourly Rate: $48.35 $68.48/hour
- Reporting To: YWHO Manager & Primary Care Manager
Key Responsibilities
- Provide acute episodic and comprehensive primary care to youth (ages 1225).
- Conduct full health assessments, including mental health, physical, and social histories.
- Diagnose and manage health conditions, prescribe medications, and order/interpreting diagnostic or lab tests.
- Deliver care grounded in mental health promotion, harm reduction, and trauma-informed principles.
- Integrate psychotherapeutic approaches into primary care practice and contribute to evidence-informed treatment planning.
- Collaborate closely with an interprofessional team to ensure coordinated, holistic care.
- Participate in complex care planning, mentorship, and continuous quality improvement initiatives.
- Contribute to clinical leadership, research, and program development to strengthen mental health service delivery for youth.
Qualifications & Experience
- Master of Nursing (MN or equivalent) with a Primary Health Care Nurse Practitioner (NP-PHC) focus.
- Registered Nurse (Extended Class) with the College of Nurses of Ontario (CNO) in good standing.
- Completion of controlled substances prescribing education (per CNO requirements).
- Demonstrated experience providing care to youth and/or young adults in a mental health or community healthcare setting.
- Proven understanding of mental health, substance use, trauma, and social determinants of health.
- Skilled in interprofessional collaboration and evidence-informed practice.
- Strong communication, assessment, and organizational skills.
- Ability to work some evening and weekend hours as required.
Assets / Nice-to-Haves
- Experience with Measurement-Based Care, research, or quality improvement projects.
- Familiarity with anti-racist, anti-oppressive (ARAOP) and trauma-informed care frameworks.
- Leadership or mentorship experience within a clinical setting.
