Nurse Practitioner Hepatitis C (HCV) Program

About the Opportunity
We are seeking a compassionate and motivated Nurse Practitioner (NP) to support a community-based Hepatitis C (HCV) Program in Scarborough. The program, delivered in partnership with the Ministry of Health, provides accessible care, education, and treatment to individuals living with or at risk of Hepatitis C. This is a dynamic opportunity to make a meaningful impact within diverse and vulnerable populations through outreach, primary care, and health promotion.
Position Details
- Employment Type: Full-Time, Permanent (35 hours/week)
- Location: Community-based; multiple sites within Scarborough
- Hourly Rate: $51.29 $72.65/hour
- Reporting To: Interprofessional Primary Care Manager
Key Responsibilities
- Deliver comprehensive, client-centred care to individuals living with or at risk of Hepatitis C.
- Provide assessment, diagnosis, treatment, prevention, and education related to HCV.
- Conduct health teaching, counselling, and follow-up support on treatment administration and side-effect management.
- Collaborate with physicians, nurses, and allied health professionals as part of an interprofessional team.
- Participate in case conferences, peer reviews, and quality improvement initiatives.
- Provide community education sessions and outreach as appropriate.
- Maintain accurate and confidential clinical records in compliance with CNO standards.
- Build and maintain professional links with external agencies, colleges, and universities.
- Cover on-call shifts within the broader primary care team as required.
Qualifications & Experience
- Masters Degree in Nursing with a Primary Health Care Nurse Practitioner (NP-PHC) designation.
- Current registration in good standing with the College of Nurses of Ontario (Extended Class).
- Member of the Nurse Practitioners Association of Ontario (NPAO) with professional liability coverage.
- 35 years NP experience in community or inter-professional primary care preferred.
- Demonstrated ability to work independently with strong organizational and clinical skills.
- Excellent interpersonal and communication abilities, with a collaborative approach to care.
- Valid Ontario drivers licence and access to a vehicle (required for community work).
- Current CPR HCP level certification.
Assets / Nice-to-Haves
- Membership in RNAO or the Canadian Association of Hepatology Nurses.
- Experience in program development or health promotion initiatives.
- Multilingual ability relevant to the Scarborough community.
- Experience working with marginalized, high-risk, or vulnerable populations (e.g., those experiencing homelessness, substance use, or chronic illness).
- Openness to mentoring students and participating in professional development.
