About the Role
Prosperity Workforce Solutions is hiring on behalf of a community health organization in Scarborough seeking a passionate Nurse Practitioner (NP) to support a Youth Wellness Hub (ages 12–25).
This role focuses on delivering episodic primary care, mental health, and substance use support within a highly collaborative, interdisciplinary model. You will play a key role in improving access to low-barrier, youth-centered care, working alongside a diverse care team to support the overall health and well-being of young people in the community.
Key Responsibilities
- Provide comprehensive, episodic primary care to youth aged 12–25
- Conduct detailed health assessments, including medical, mental health, and social histories
- Assess, diagnose, treat, and manage acute and stable chronic conditions
- Order and interpret diagnostic tests and prescribe treatments within scope of practice
- Deliver health education and preventative care, integrating mental health and substance use support
- Collaborate with an interdisciplinary team including counsellors, therapists, peer support workers, and service navigators
- Participate in complex care planning and case discussions
- Support measurement-based care and quality improvement initiatives
- Provide consultation and clinical guidance to team members as needed
- Build strong connections with community partners and referral networks
Qualifications
Education & Registration:
- Master's degree as a Nurse Practitioner (Primary Health Care)
- Current registration in good standing with the College of Nurses of Ontario (Extended Class)
- Professional liability coverage (NPAO membership preferred)
- Current CPR (HCP level) certification
Experience
- Experience as a Nurse Practitioner in primary care, youth health, or community settings preferred
- Experience supporting mental health and substance use needs is strongly preferred
- Experience working with youth and diverse populations is an asset
- New and early-career NPs with relevant clinical exposure are encouraged to apply
