Primary Care Physician – Part-Time (17.5 hrs/week)

Role Summary:

Prosperity Workforce Solutions is hiring a Part-Time Primary Care Physician on behalf of a community-based healthcare organization. The Physician will provide comprehensive, patient-centered primary care with a strong focus on underserved, marginalized, and medically complex populations.

This role operates within an interdisciplinary care model and requires a provider who is comfortable managing complex clinical and social needs while contributing to team-based care planning and community health initiatives.

Key Responsibilities:

  • Deliver full-scope primary care (acute, chronic, preventive) to individuals and families
  • Manage complex patients with co-morbidities, mental health, addictions, and social vulnerabilities
  • Collaborate closely with an interdisciplinary team (NPs, RNs, social workers, allied health)
  • Support case management and care planning for high-needs patients
  • Provide clinical leadership, consultation, and mentorship to team members
  • Participate in development of care pathways and medical directives
  • Engage in outreach, health promotion, and preventive screening initiatives
  • Apply trauma-informed, harm-reduction, and culturally safe care approaches
  • Support and precept medical and NP learners
  • Contribute to quality improvement and program evaluation initiatives
  • Provide on-call support to NPs and participate in extended hours coverage as required

Must-Have Requirements:

  • MD or equivalent medical degree from a recognized institution
  • Licensed and in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
  • Certification with the College of Family Physicians of Canada (CCFP) (or equivalent)
  • Active membership with CMPA
  • Strong competency in comprehensive family medicine
  • Experience working in team-based or community healthcare settings

Preferred / Nice-to-Have:

  • 3–5 years of experience in a CHC, FHT, or interdisciplinary care model
  • Experience working with underserved or marginalized populations
  • Background in mental health, addictions, or complex care
  • Experience with EMRs (PS Suite preferred)
  • Multilingual abilities
